Vandalia Health Network Partners with Unite Us to Address Social Determinants (SDoH) of Health and Improve Health Equity

Vandalia Health Network (VHN), a united healthcare system, and Unite Us, the nation’s leading software company enabling cross-sector collaboration, are partnering to connect the identification of needs to efficient resource access. The partnership will analyze specific needs within the West Virginia community and pinpoint the most significant impact areas.
VHN aims to eliminate socioeconomic barriers and improve patients' health equity and clinical outcomes. Currently, VHN serves over 100,000 patients, all experiencing different barriers in their health care. By engaging with local communities and collaborating with Unite Us, we can ensure quality improvement in a patient's general care.
In West Virginia, Unite Us connects people to much-needed resources and services, such as assistance with housing, shelter, food, and individual/family support. Partners in the network are connected through Unite Us’ shared technology platform, enabling them to send and receive secure electronic referrals, address people’s social needs, and improve community health.
“Although a healthcare provider can control a person's medical care, they do not necessarily control the environmental factors that affect their health,” said Michelle Coon, President of VHN. “Partnering with Unite Us to address these issues with the external community, state, and national partners makes sense. Healthcare providers should consider ‘thinking outside the box’ regarding partnerships to address SDoH,” added Coon.
Together, Vandalia Health Network and Unite Us will securely connect people in need with community-based resources that can otherwise be difficult to navigate. The network care management team can securely access and refer people to needed services while monitoring progress and measuring outcomes.
